Students must have a physical examination on file in the athletic office, before they may participate in athletics. A physical dated on or after April 15, 2020 is valid for the 2020-2021 season to participate in athletics.
Due to the circumstances of the pandemic, physicals that were turned in last year for the 2019-2020 sports seasons are valid for the upcoming 2020-2021 seasons. Students who plan on using their physical from last year will need to turn in the MHSAA Health Sports Questionnaire that can be found on their Final Forms account under the MHSAA PPE Physical (for Physicians) form. A hard copy will need to be turned in before students are allowed to practice/tryout.
6th graders, transfer students, or students who did not play any sports last year but want to this year will need to turn in an updated physical dated after April 15, 2020 in order to participate.
Online payment, cash, or check payable to WEST OTTAWA PUBLIC SCHOOLS in the amount $35.00 is due on or before the first day. Until this is received, athletes will not be allowed to participate. The Panther Fund can provide assistance with start-up costs for student athletes who want to participate but cannot afford those costs.
Students who paid the insurance fee for the 2020 spring season that was cancelled will have their payment roll over for the fall.
All athletes will need to be registered on Final Forms prior to the start of practice. Head to westottawa-mi.finalforms.com to start the registration process if you don’t have an account already. After their physical is turned in, they are registered on Final Forms and eligibility is verified athletes will be permitted to practice/tryout.
