Fall season is fast approaching.  Just a little reminder of the items that need to be turned in prior to the first day of practice.

1. Sports Physical.  An athlete cannot participate until this is turned in.

2. The Student Handbook is signed and turned in.

3. Concussion Form.  The new state law went into effect on June 30th.  Students and parents need to read the education materials and sign-off and return the concussion form to the Athletic Office.

4.  Mandatory ImPACT testing: If your student is an incoming freshmen or upper class student that has not taken the ImPACT baseline test, please schedule this soon.

This form has information on what ImPACT is.  We do not need a signed sheet back due to it being required by all athletes in high school.  PermissionForm
